Model Release & Consent Form — Music Video for “Dance While The Windows Open”
About this form
This form gives Hamid Najafi ("the Producer") permission to use the video clip and personal information you submit for the crowd-sourced music video project described below.
Please read it before you upload your clip. Submitting your video constitutes your agreement to these terms, even without a physical signature, unless a signed copy is specifically requested.
1. The Project
You are submitting a short video (up to 60 seconds) of yourself dancing — on a street corner, at home, in your office, on public transit, or in another setting of your choosing — for use in an official music video for the song “Dance While The Windows Open,” written and produced by Hamid (also known professionally as Zubin Cesar).
Clips from many contributors around the world will be edited together into a single video.
2. Grant of Rights
By submitting a video, you grant the Producer a worldwide, royalty-free, irrevocable (subject to Section 7) license to:
• Use, reproduce, edit, crop, color-correct, slow down, speed up, or otherwise modify your submitted footage;
• Combine your footage with other contributors’ footage, the song, graphics, and other creative elements;
• Include your name, city/location, and likeness as they appear in the footage; and
• Distribute and publicly display the resulting music video on YouTube, streaming platforms, social media (Instagram, TikTok, etc.), the Producer’s and Zubin Cesar websites, press and promotional materials, and any future re-releases or edits of the video.
3. No Obligation to Use
The Producer is not obligated to include your specific clip in the final video. Submission does not guarantee inclusion, and the Producer may select, omit, or re-edit any footage at their sole discretion.
4. Compensation
You understand this is a fan/community collaboration and that no payment, royalty, or other compensation is owed to you for your appearance or footage, unless separately agreed to in writing.
5. Your Content and Rights
You confirm that:
• You are the person appearing in the video (or, if others appear alongside you, you have their permission to include them, and by submitting you confirm they also agree to these terms);
• You have the right to grant this license (e.g., the location and any music playing in the background does not infringe on anyone else’s rights); and
• The footage does not contain anything unlawful, obscene, or infringing on a third party’s rights.
6. Minors
If the person appearing in the video is under 18, a parent or legal guardian must complete the signature section below on their behalf, confirming consent for the minor’s participation and likeness to be used as described in this form.
7. Withdrawing Consent
You may request that your clip be removed from future edits or re-releases of the video by contacting the Producer directly.
Removal cannot be guaranteed once the video has already been published or distributed, but reasonable efforts will be made to honor removal requests going forward.
8. Release of Claims
You release the Producer and Zubin Cesar from any claims relating to the use of your footage as described in this form, including any claims of defamation, invasion of privacy, or infringement of moral rights, to the extent permitted by applicable law.
